Abstract: Foreign body aspiration is commonly encountered in children. In cases that foreign body does not disturb respiratory physiology, clinical and radiological diagnosis may be delayed leading to severe complications. Four cases with aspiration of a foreign body not obstructing ventilation and without typical clinical and radiological findings are discussed.

Abstract: Purine Nucleoside Phosphorylase (PNP) deficiency is a rare autosomal recessive metabolic disorder. In PNP-deficiency disorder, the deficient enzyme leads to accumulation of toxic metabolites, especially in lymphocytes and the metabolites exert toxic effect on T-cell generation. Purine nucleoside phosphorylase deficiency causes decreased numbers of T cells and lymphopenia. The patients suffering from PNP-deficiency may be admitted with recurrent infections, as well as neurological and autoimmune findings. We hereby presented a case admitted with the symptom of hematuria in which we established the diagnosis of PNP-deficiency early on the basis of detection of lymphopenia and low level of uric acid.
Atopy frequency in children with gastroesophageal re ux

Abstract: Objective: Gastroesophageal re ux disease (GERD) may be secondary to infectious, allergic, neurological, systemic and metabolic diseases. In this study, we aimed to evaluate the frequency of atopy in children that is followed-up in our hospital with GERD. Material and Methods: Children who had presented to Dr. Beh et Uz Children Hospital, Allergy and Gastroenterology, Hepatology and Nutrition outpatient clinics between September 2007 and February 2008 with vomiting, abdominal pain, cough or wheezing and had diagnosed as GERD in scintigraphy were included. Presenting symptoms, duration of symptoms, received treatment, response of the cases to the treatments were recorded. Total eosinophil count, total immunoglobulin E (IgE) levels, presence of food mixture (fx5) and inhalant specific IgE (sIgE) were investigated. Skin prick test was performed to all cases with positive food or inhalant sIgE. Children with positive skin prick test were accepted as “atopic”. Atopy rate was compared to the values in Turkish children. Children who did and did not respond to treatment were compared with regard to these parameters. Results: A total of 59 children, of which 25 (42%) were girls; with a mean age of 4.7±2.9 years were included in the study. Children who did and did not respond to treatment were not different with regard to parameters mentioned above. Atopy rate (10%) in children with GERD was found similar to atopy rate (8.8%) determined in children from zmir region. Conclusion: No increase in atopy rate was found in children with GERD. No relation was determined between response to treatment and atopy.
Foreign body aspiration mimicking croup syndrome: Case report

Abstract: The term croup refer to a group of conditions involving in ammation of the upper airway that leads to a cough that sounds like a bark, difficulty breathing, and a grunting noise or inspiratory stridor during breathing. The most common characteristics of these ailments are upper airway obstruction. The obstruction level determines the degree of respiratory distress and rarely causes respiratory failure as well. Except for croup the other causes of acute upper airway obstruction in children are inhalated foreign bodies, retropharyngeal abscess, bacterial tracheitis and epiglottitis. Two cases presented to the emergency department with the diagnosis of croup syndrome. A 9 month old girl had an incremental stridor which was lasting for two months. She underwent rigid bronchoscopy with removal of an undefined foreign body organized at vocal cord level. The other case, a 19 month old boy, was admitted with cardiac arrest. On observation after a choking episode he ejected a piece of peanut spontaneously.
Clinical evaluation of World Health Organization criteria proposed for the diagnosis of tuberculosis in children

Abstract: Objectives: Because of the difficulty in confirming childhoodtuberculosis, the World Health Organization developeda scale based on clinical parameters. In our studythe utility of this scale was evaluated.Materials and methods: Patients whom were admittedto the Dr. Beh et Uz Children Hospital due to the complaintsof chronic cough, weight loss and/or close householdcontact with adult tuberculosis were included. Patientswere evaluated in according to the history, physicalexamination and/or chest radiography, PPD and theresults of the cultures obtained from fasting gastric juice/sputum and implanted on Bactec and Lowenstein-Jensenculture medium.Results: Totally, 108 patients were included in this study.According to World Health Organization criteria, 24 cases(22.2%) were classified as suspicious, 80 cases (74.1%)as probable and 4 of them (3.7%) as definite tuberculosis.Lymphodenopathy and atelectasis in chest radiographyand in addition lymphodenopathy, bronchiectasis andcalcification in thorax computed tomography were morefrequently seen in the PPD positive group compared withthe PPD negative group (p=0.012, p=0.02). Anti-tuberculosistreatment were not given for twenty four patientswho were included in suspicious group and tuberculosiswere not seen in any of these patients during their sixmonth follow up period. During the six months follow-upof the probable and definite tuberculosis groups, clinicalfindings improved in all of the patients who were treated.Conclusion: World Health Organization diagnosis criteriawere considered to be useful for the diagnosis of tuberculosisin children. Abstract: Idiopathic pulmonary hemosiderosis (IPH) is an uncommon chronic disorder in children. It is characterized by recurrent pulmonary hemorrhage and may result in hemoptysis and pulmonary insufficiency. The most common hematologic manifestation of IPH is iron deficiency anemia. The etiology of IPH is not known and its diagnosis may be difficult due to the variable clinical courses. The most helpful signs for identifying IPH are iron deficiency anemia and recurrent or chronic cough, hemoptysis, dyspnea, wheezing. We report here 5 pediatric cases of IPH presenting with iron deficiency anemia and without pulmonary symptoms. Mean corpuscular volume was low in all patients; iron was low in 4 out of 5 cases; total iron binding capacity was high in all of them; ferritin was low in 3 patients. At follow up, none of them had responded successfully to the iron therapy. Although they didn’t present with pulmonary symptoms, chest radiographs incidentally revealed diffuse reticulonoduler shadows in all of them. Computed tomography revealed diffuse ground-glass opacities, consolidation, increased density. The diagnosis was confirmed by the detection of hemosiderinladen macrophages in bronchoalveolar lavage fluid and gastric aspirate. If patients with iron deficiency anemia don’t respond to iron therapy, they should be examined for IPH. Chest radiographs should be taken even in absence of pulmonary symptoms. Early diagnosis is important for a timely management of IPH.
The Efficacy of Specimen Radiography in Clinically Occult Breast Lesions

Abstract: Objectives: The aim of this study was to evaluate the efficacy of specimen radiography for clinically occult breast lesions.Patients and Methods: We retrospectively evaluated specimen radiographs of 215 patients (mean age 48.64±8.4 years; range 28 to 75 years) whose lesions were detected by mammography and/or ultrasonography and localized by wire. Preoperative wire localization was performed under the guidance of ultrasonography in 59 cases (27.4%) and stereotactically in 156 cases (72.6%). The findings of specimen radiography were compared with those of histopathologic examination.Results: Specimen radiographs showed lesions in all but three cases. In these three cases, no breast lesions were found in control mammographies.Conclusion: Breast biopsy specimen radiography is a safe method to ensure complete excision of clinically occult breast lesions.

Abstract: We generalize some homotopy calculation techniques such as splittings and matching trees that are introduced for the computations in the case of the independence complexes of graphs to arbitrary simplicial complexes, and exemplify their efficiency on some simplicial complexes, the devoid complexes of graphs, which are simplicial complexes parametrized by graphs. Additionally, we compute the homotopy type of dominance complexes of chordal graphs.
